Ginecomastia reprezinta o afectiune ce are ca rezultat dezvoltarea sau marirea in exces a sanului la barbati. Cauzele aparitiei ginecomastiei pot fi diverse: ereditate, dereglari hormonale, folosirea unor medicamente cum ar fi prednisonul, cimetidina, antiepilepticele – fenitoina, etc. Ginecomastiile pot aparea la unul sau ambii sani, putand fi catalogate ca ginecomastii unilaterale si, respectiv, ginecomastii bilaterale.

Tipuri

Ginecomastia apare de obicei sub forma de:
-tesut adipos (grasime) la nivelul sanului.
-exces al glandei sanului.
-sau o combinatie intre cele doua.

Ca raspandire, ginecomastiile pot afecta atat adolescenti, cat si adulti. La adolescenti, in majoritatea cazurilor, ginecomastia dispare spontan pana la varsta de 18 ani. In general, ginecomastia afecteaza intre 40-60% dintre barbati. In functie de stadiul afectiunii, ginecomastia poate fi impartita in 3 grade:

Gradul 1
– exces moderat de tesut glandular si grasime.
Gradul 2
– exces mare de tesut glandular si de tesut adipos (grasime) cu sau fara exces tegumentar
Gradul 3
– exces exagerat de tesut glandular si de grasime cu exces tegumentar vizibil.

Inainte de operatie

Inainte de orice operatie de ginecomastie trebuiesc efectuate cel putin o examinare medicala a doctorului chirurg estetician si urmatoarele analize:
-> teste sanguine, inclusiv probe hepatice si dozarea hormonilor;
-> teste urinare;
-> consult endocrinologic;
->mamografie si ecografie de tesut adipos

Se recomanda oprirea fumatul inainte cu 2 saptamani de operatie si de asemenea, evitarea anumitor medicamente cum ar fi aspirina, anti-inflamatoare nesteroidiene, vitamina E.

Operatia

Operatia de ginecomastie se practica sub anestezie generala si dureaza aproximativ 2 ore. Procedura chirurgicala se poate prelungi in unele cazuri, durata operatiei de ginecomastie depinzand de tehnica efectuata:
-lipoaspiratie in cazul excesului de grasime.
-excizia glandei si a tesutului adipos.

Inciziile sunt diferite in functie de gradul de ginecomastie:
– periareolara (in jurul areolei)- pentru ginecomastii usoare spre moderate;
– incizie verticala si periareolara;
– incizie in T inversat pentru ginecomastie foarte severa.

In cazurile in care se apeleaza la ginecomastie cu liposuctie, datorita tehnologiilor utilizate – Vaser Lipo – ginecomastia nu mai poate reaparea, deoarece numarul celulelor adipoase nu se mai reface in zona liposuctionata. In astfel de cazuri, rezultatul este unul eficient iar retractia tegumentara este mult mai rapida.

Dupa ginecomastie

Dupa ginecomastie este necesar sa purtati un bandaj elastic (vesta elastica) timp de 2 saptamani zi si noapte. Dupa aceasta perioada, pentru o vindecare mai rapida, este necesar a se purta vesta inca 2 saptamani, timp de 12 ore pe zi.

In primele 7 – 10 zile de la ginecomastie pacientul va prezenta edem si echimoze, acestea disparand treptat.
Activitatea se poate relua rapid, in 2-3 zile dupa ginecomastie, insa sportul sau activitatile ce presupun efort fizic trebuiesc intrerupte cel putin 2-3 saptamani.

Dupa operatie se recomanda evitarea expunerii la soare timp de 6 luni.
